Tbh I wouldn't pay to have iptv on your iPad due to the fact of what if you don't like the app or viewing on your iPad is he going to refund you and any provider wouldn't charge to put it on a separate device

A lot of suppliers would charge the reseller cost of adding the service to the app... Lots of apps on iPad require the details input via mac address, and have a charge per mac... This is to save seller having to give url of server...
